Immunizations are administered through the age of 18

Walk-In Basis - Monday through
Friday - During regular office hours |

Immunization Requirements - Kansas Children are required to be
fully immunized to attend school.
Fully immunized includes the following:

| Five (5) doses of
diphtheria, tetanus and pertussis * vaccine |
| Four (4) doses of
polio vaccine |
| Two (2) doses of
measles vaccine |
| Two (2) doses of mumps
vaccine |
|
Two (2) doses of rubella
vaccine |
|
One (1) dose of Varicella to prevent
chickenpox OR history of disease. Two
doses will likely be required by 2008 for school entry. |
| Three (3) doses of
Hepatitis B vaccine. |

| INFLUENZA
Recommendation from CDC: Children between 6 months of age
and 23 months should receive the annual influenza vaccine,
usually given from October through December of each year, in
order to prevent influenza and its complications among
infants. |

| * A tetanus-diphtheria
booster is necessary on entrance to high school, depending on
when the last dose was received. |
