Immunizations

IMMUNIZATIONS

 

Walk-In Basis - Monday through Friday - 

During regular office hours

Immunization Schedule  for Children   0 to 6 years of age 

Immunization Schedule for Children    0 to 6 years of age    Spanish

Immunization Schedule for Youth age 7 to 18

Immunization Schedule for Youth age 7 to 18  Spanish 

Kansas Children are required to be fully immunized to enter or attend school. 

Kansas Certificate of Immunizations 

Diphtheria, Tetanus and Pertussis: 5 doses of vaccine by  Kindergarten
Tetanus, Diphtheria, and Pertussis Booster (TDaP):   At age 11-12
Polio: 4 doses of vaccine
MMR:  2 doses of measles, mumps, and rubella vaccine

Varicella:  2 doses of vaccine to prevent chickenpox .

Hepatitis B: 3 doses of vaccine.

Hepatitis A:  2 doses required for children less than 5 yrs of age in early childhood programs.
 
INFLUENZA Recommendation from CDC: Children between 6 months of age and 18 years should receive the annual influenza vaccine to prevent influenza and its complications, unless contraindicated.
* A tetanus-diphtheria-pertussis booster is necessary on entrance to high school, depending on when the last dose was received.
